JOB TITLE: Family Physician

PROGRAM AREA: Clinical

EMPLOYMENT STATUS: Locum – 1 year

HOURS/WEEK: 28 hours per week

 

 

What is the Guelph CHC?

The Guelph CHC is located in the vibrant city of Guelph, Ontario, and provides innovative primary care services and community health programs using an interdisciplinary approach.  We have innovative client-centered services delivered through the use of interdisciplinary teams.   Our clinical team consists of family physicians, nurse practitioners, and registered practical nurses, who work collaboratively with Guelph CHC community health workers, including social workers, dietitians, outreach workers, and cultural interpreters.

 

The Guelph Community Health Centre works within a determinants of health framework which emphasizes health promotion and education, disease prevention, anti-oppression, and recognition of special risk factors (social, physical, psychological, etc.) which may affect the health of clients.

 

This position is at the Shelldale location of the CHC.

Primary Responsibilities:

  • Provide primary care services, including comprehensive assessment and treatment, supportive client counseling, prenatal and neonatal care, and clinical consults with Nurse Practitioners
  • Work within a determinants of health framework which emphasizes health promotion and education, disease prevention and recognizes the special risk factors (social, physical, psychological etc.) which may affect the health of clients
  • Participate in the Centre’s on call service as directed (consisting of courtesy hospital visits and phone calls in rotation with other physician colleagues) and occasional home visits as necessary
  • Ensure appropriate, comprehensive treatment is delivered by maintaining complete and accurate medical records, participating in chart reviews, and discussion of problem cases and answering client inquiries
  • Maintain professional development through continuing education, including Practice Based Learning opportunities
  • Be an active member and contribute to assigned interdisciplinary teams

Qualifications:

·         Licensed Family Physician, as per the requirements of the Ontario College of Physicians and Surgeons, with CCFP certification

·         Possess or is eligible to obtain required malpractice coverage with CMPA (this coverage is eligible for reimbursement)

·         Experience working with communities and individuals who face barriers accessing traditional health care institutions (such as individuals facing poverty, language barriers, discriminations, mental health and substance use issues, etc.)

·         Excellent communications skills, both verbal and written, along with the ability to prioritize and multi-task

·         Proficient in the use of computers and various other software applications including Word, Outlook, and electronic health records

·         Ability to work interdependently and respectfully in an interdisciplinary team environment

·         Previous experience in community health/social services and/or a values based organization is an asset

·         Proficiency in other languages is an asset

JOB TITLE: Health Services Receptionist

PROGRAM AREA: Clinical

EMPLOYMENT STATUS: Part-time, ongoing

HOURS/WEEK: 19 hrs/week + Relief

 

The part-time Health Services Receptionist will use their excellent communication and customer service skills to greet and direct visitors and clients, handle incoming phone calls, book patient appointments and referrals.  They will also need to assist in the smooth operation of the clinic by maintaining an orderly and efficient environment, scheduling health services providers’ time, working with Electronic Client Records, filing/scanning documents, retrieving charts/reports, distributing mail and faxing confidential documents. This position will include regular part time scheduled hours at both our downtown and Shelldale locations as well as relief hours with availability to work daytime and evening hours.   

You must have a post secondary education in a discipline pertinent to the job or a certificate that focuses on medical/office administration, along with 1-3 years of relevant experience in a medical reception environment or community setting. Your strong interpersonal and empathetic skills, combined with your experience working with culturally, economically and socially diverse clients, make you the ideal candidate. Intermediate to advanced skills in Word, Excel and Outlook are considered assets. Experience working with Electronic Health Records is essential. You have a clear understanding of the determinants of health, especially as they relate to our priority groups and have demonstrated the ability to work both independently and as part of a multidisciplinary team. JOB TITLE: Early Literacy Specialist

HOURS OF WORK:  17.5 hours per week, contract 1 year (mat leave) 

LOCATION: Stone Road Mall  

 

Reporting to the Ontario Early Years Program Manager, the Early Literacy Specialist will support early literacy as well as numeracy throughout Wellington County by providing resources, workshops, seminars and consultations for parents, caregivers, community organizations and early childhood professionals.

QUALIFICATIONS:

 

  • Member of the College of Early Childhood Educators or graduation from a recognized post-secondary institution in child development, library sciences, health promotion or community development or an acceptable level of education, skills and experience
  • Three – five years of experience in child and family services
  • Demonstrated knowledge of child development and parenting and/or early literacy
  • Demonstrated program planning, coordination and evaluation skills
  • Ability to set goals and get results within a set timeline
  • Ability to multitask and work independently
  • Effective written and oral communication skills
  • Effective time management, decision making and organizational skills
  • A good team member and supportive of interdisciplinary practice
  • Demonstrated knowledge of MS Office
  • High level of confidentiality
  • Flexibility towards change
  • Valid MTO driver’s license and access to reliable transportation

 

WORKING ENVIRONMENT 

  • Some evening or week-end work required 
  • 1-3 hour intervals spent sitting at desk or in meetings

PRINCIPAL JOB DUTIES AND RESPONSIBILITIES

  • Create kits for parents to borrow to assist them in fostering readiness to learn skills in their children.
  • Deliver workshops pertaining to literacy topics to parents/practitioners.
  • Conducting ‘Train-the Trainer’ sessions with early years practitioners and parents in the community.
  • Collaborate with libraries, museums, and other recreation/cultural programs, parenting programs, adult education and literacy programs.
  • Work with the Data Analysis Coordinator in reviewing community needs using the Early Development Instrument, Kindergarten Parent Survey and other local assessment tools. 
  • Initiate regular community forums that bring together early years practitioners in all sectors to coordinate literacy efforts and ensure parents and other child care providers participate in and benefit from early literacy activities in their community.
  • Collaborate with Quality Child Care Initiative to enhance early literacy in local child care sector.
  • Establish appropriate linkages with their local:

  • Preschool Speech and Language/Healthy Babies, Healthy Children staff
  • School Board(s) and Parent Council(s)
  • Child Care Centers

JOB TITLE:  Director, Programs and Services     

EMPLOYMENT STATUS: Full-Time

HOURS/WEEK: 35 hr/week

The Director of Programs and Services brings collaborative leadership, team building, planning and program evaluation skills to support the strategic alignment and effective management of our interdisciplinary team of primary health care and early child development professionals.   Reporting to the Executive Director, this position ensures the efficient, effective and responsive service delivery in a rapidly changing primary health and community services environment.  

As Director, Programs and Services, you will support the efforts of Guelph CHC's senior management team to manage change, deliver person-centred services, and generate deeper community impact.  A dynamic leader, your communication and team-building skills will support internal staff and external agencies and stakeholders to pursue innovative solutions which improve services and programs.   

Combining excellent strategic planning, coaching and communication skills, you will provide strategic direction to three distinct Program Areas -- Clinical Services, Community Health, and Early Child Development.  You have a strong track record of team leadership, with proven strengths in mentoring and coaching,  program planning, performance measurement and the development of collaborative service partnerships. This position oversees portfolio budgets, risk management and client safety, staff development and training needs.

You have a post-secondary degree in health services or a related field with at least 5 years of experience in a community based or health services agency that included managing programs and supervising staff.   You have experience and success in leading a team along with experience in coordinating, planning, delivering and evaluating programs. You have an excellent understanding of the determinants of health, especially as they relate to our priority populations. You understand and are committed to the Mission, Vision and Values of the Guelph CHC.  You can prioritize and manage complex work assignments.  You have a valid Class G driver’s license, personal transportation, and a clear driving record.

JOB TITLE: Senior Data Management Coordinator

PROGRAM AREA: Quality Improvement & Information Management (QIIM)

EMPLOYMENT STATUS: Part-Time, ongoing

HOURS/WEEK:  24.5-28 hrs/week

 

You will work closely with the other members of the Quality Improvement and Information Management Team and across Guelph CHC to provide access to high quality data, data analysis expertise, program planning, and electronic health records support.  As the senior DMC, you will be responsible for overseeing the work of the associate DMC, project coordination and reporting, and scheduling support.

 

Combining excellent interpersonal, organizational and communication skills with strong data quality management expertise, you will develop, generate, analyze and interpret data reports from our client records system, support staff in the effective utilization of the electronic client records, and assume responsibility for our health records database application. Additional responsibilities include data extraction and report generation for government funding agencies, and coordination of database systems support with Association of Ontario Health Care Centres and external vendors. 

 

You have a university degree in health administration and planning, health informatics, information management, or equivalent, with over 5 years data management experience in a community-based, multidisciplinary health or social services environment.  Your training;  experience with SQL databases/reporting tools, and statistics; ability to adapt to changing priorities;  and analytical problem solving skills are coupled with your ability to effectively work with a wide range of health professionals, support staff, and external agencies to prioritize, mange and resolve multiple requests in a timely manner.  The ability to work flexible hours, including some weekends and evenings, a valid Class G driver’s license, personal transportation, and a clear driving record will be required.
