JOB TITLE: Human Resources Coordinator
EMPLOYMENT STATUS: Contract (12 Months)
HOURS/WEEK: 35 hours per week
As a member of the Business Support Team, the Human Resources Coordinator participates in the development, implementation and monitoring of employment practices, policies, systems and procedures such as; recruitment, orientation, performance management and related ABCs. This role is also responsible for: 1) ensuring government, legislative and contractual compliance, as it relates to Human Resources, 2) development and implementation of systems of record keeping, data collection, reporting and information distribution such as; employee documentation and files, staffing logistics and HR reporting, 3)Employee relations, communication and support, 4) ensuring compliance and understanding of the ONA Collective Agreement, 5) administering employee benefit packages, salary and hourly payroll administration including government reporting.
You have an Undergraduate degree in a relevant discipline or a combination of diploma coupled with three to five years of progressive human resources generalist experience. Certified Human Resources Professional (CHRP) would be considered an asset. You have experience developing Human Resources policies and a clear understanding of related legislation and experience in administering payroll. You are able to set goals and get results within a set timeline and are able to multitask and work independently. You have effective written and oral communication skills and effective time management, decision making and organizational skills. You are a good team member and supportive of interdisciplinary practice and are able to exhibit a high level of confidentiality and flexibility towards change.

JOB TITLE: Nurse Practitioner (Downtown & Shelldale Clinic)
PROGRAM AREA: Clinic
EMPLOYMENT STATUS: Contract (13 months)
HOURS/WEEK: 35
The Guelph CHC is a community based organization in Guelph, Ontario currently seeking a Nurse Practitioner. As a member of our Clinical team, the Nurse Practitioner will work with Guelph CHC’s multi-disciplinary team and provide Primary Health Care and Health promotion/heath education services.
POSITION DUTIES AND RESPONSIBILITIES:
- Perform assessment, diagnosis, treatment, health education/counseling and other clinical activities according to the College of Nurses of Ontario standards of practice or Registered Nurses in the Extended Class (NPs)
- Contribute to chart documentation, participate in chart reviews and case conferences, and make referrals.
- Participate in the development, implementation, monitoring and evaluation of treatment, education, counseling and health promotion programs and services for individuals, families and the community.
Primary care:
i. Apply the determinants of health model and the Ontario College of Nurses Standards of Practice for RN (EC) to the provision of well person, episodic and stable chronic illness care to people of all ages and diverse backgrounds.
ii. Use of office assessment, telephone advice, home visits, satellite, specialty clinics and a variety of other approaches in the delivery of primary care.
iii. Ensure appropriate, comprehensive treatment is delivered by maintaining complete and accurate documentation, participating in chart reviews and case analysis.
iv. Respond to patient and participant inquiries by directing them to the appropriate service or treatment.
v. Apply and participate in the development of appropriate professional and organizational policies, protocols and procedures.
vi. Understand the role of, refer appropriately to and communicate effectively with other health professionals and community services.
vii. Understands the special health needs of priority populations
Community Development:
i. Provide support and facilitate the development of life skills in patient/participants through individual and group interaction.
ii. Engage individual residents in the process of working with community partners and other health professionals, including volunteer and committee work.
iii. Engage community partners in working with individual participants/patients.
iv. Supervise students and participate in the orientation of new staff members.
Other:
i. Participate in individual and clinical team professional development
ii. Other duties as assigned.
REQUIREMENTS:
- Baccalaureate in Nursing and Post Graduate Nurse Practitioner Education
- Current College of Nurses of Ontario Certificate of Competence
- Extended Class Certification
- Minimum 2 years experience primary health care/community health
- Interest and experience in working in a range of community based settings with a variety of populations including the homeless, individuals with mental health challenges, new immigrants
- Skills/experience to fulfill major functions of role: wellness care; assessment, diagnosis and management of episodic illness; monitoring chronic disease; and group work
- Ability to work flexible hours including evenings (one evening per week)
In addition to the clinical skills, candidates for this position will have: excellent communication and problem solving skills; excellent organizational, interpersonal, decision making and time management skills; proficiency in the use of computers and software applications, including computer documentation.

The Guelph Community Health Centre is looking to fill the following Two (2)Full-Time position: Family Physician, MD (full-time & Maternity Leave Coverage)
The Guelph CHC provides innovative primary health services in an exciting and dynamically changing work environment.
Using an interdisciplinary approach, you will maximize your skills in delivery of the following:
- Provide primary health care services, including comprehensive assessment and treatment, supportive client counseling, prenatal and neonatal care, and clinical consults with Nurse Practitioners.
- Work within a determinants of health framework which emphasizes health promotion and education, disease prevention and recognizes the special risk factors (social, physical, psychological etc.) which may affect the health of clients.
- Maintain professional development through continuing education, including Practice Based Learning opportunities.
You are able to work effectively, both independently and interdependently to provide comprehensive client-centred care. You possess strong organizational and analytical skills, and demonstrate a sound knowledge of the Ontario Health Care System and the Community Health Centre model of care.
You are a licensed Family Physician, as per the requirements of the Ontario College of Physicians and Surgeons, with CCFP certification.
