JOB TITLE: Family Physician

PROGRAM AREA: Clinical

EMPLOYMENT STATUS: Locum – 1 year

HOURS/WEEK: 28 hours per week

 

What is the Guelph CHC?

The Guelph CHC is located in the vibrant city of Guelph, Ontario, and provides innovative primary care services and community health programs using an interdisciplinary approach.  We have innovative client-centered services delivered through the use of interdisciplinary teams.   Our clinical team consists of family physicians, nurse practitioners, and registered practical nurses, who work collaboratively with Guelph CHC community health workers, including social workers, dietitians, outreach workers, and cultural interpreters.

The Guelph Community Health Centre works within a determinants of health framework which emphasizes health promotion and education, disease prevention, anti-oppression, and recognition of special risk factors (social, physical, psychological, etc.) which may affect the health of clients.

This position is at the Shelldale location of the CHC.

Primary Responsibilities:

  • Provide primary care services, including comprehensive assessment and treatment, supportive client counseling, prenatal and neonatal care, and clinical consults with Nurse Practitioners
  • Work within a determinants of health framework which emphasizes health promotion and education, disease prevention and recognizes the special risk factors (social, physical, psychological etc.) which may affect the health of clients
  • Participate in the Centre’s on call service as directed (consisting of courtesy hospital visits and phone calls in rotation with other physician colleagues) and occasional home visits as necessary
  • Ensure appropriate, comprehensive treatment is delivered by maintaining complete and accurate medical records, participating in chart reviews, and discussion of problem cases and answering client inquiries
  • Maintain professional development through continuing education, including Practice Based Learning opportunities
  • Be an active member and contribute to assigned interdisciplinary teams

Qualifications:

  • Licensed Family Physician, as per the requirements of the Ontario College of Physicians and Surgeons, with CCFP certification
  • Possess or is eligible to obtain required malpractice coverage with CMPA (this coverage is eligible for reimbursement)
  • Experience working with communities and individuals who face barriers accessing traditional health care institutions (such as individuals facing poverty, language barriers, discriminations, mental health and substance use issues, etc.)
  • Excellent communications skills, both verbal and written, along with the ability to prioritize and multi-task
  • Proficient in the use of computers and various other software applications including Word, Outlook, and electronic health records
  • Ability to work interdependently and respectfully in an interdisciplinary team environment
  • Previous experience in community health/social services and/or a values based organization is an asset
  • Proficiency in other languages is an asset

JOB TITLE: Health Services Receptionist

PROGRAM AREA: Clinical

EMPLOYMENT STATUS: Part-time, 4 month contract

HOURS/WEEK: 28 hrs/week + Relief

The part-time Health Services Receptionist will use their excellent communication and customer service skills to greet and direct visitors and clients, handle incoming phone calls, book patient appointments and referrals.  They will also need to assist in the smooth operation of the clinic by maintaining an orderly and efficient environment, scheduling health services providers’ time, working with Electronic Client Records, filing/scanning documents, retrieving charts/reports, distributing mail and faxing confidential documents. This position will include regular part time scheduled hours at both our downtown and Shelldale locations as well as relief hours with availability to work daytime and evening hours.   

You must have a post secondary education in a discipline pertinent to the job or a certificate that focuses on medical/office administration, along with 1-3 years of relevant experience in a medical reception environment or community setting. Your strong interpersonal and empathetic skills, combined with your experience working with culturally, economically and socially diverse clients, make you the ideal candidate. Intermediate to advanced skills in Word, Excel and Outlook are considered assets. Experience working with Electronic Health Records is essential. You have a clear understanding of the determinants of health, especially as they relate to our priority groups and have demonstrated the ability to work both independently and as part of a multidisciplinary team. JOB TITLE: Marketing & Communications Assistant - Special Projects

PROGRAM AREA: Corporate

EMPLOYMENT STATUS: Part Time, 10 month contract (possibility of renewal)

HOURS/WEEK: 14-21 (flexibility)

 

The Marketing and Communications Support position reports to the Volunteer & Resource Development Coordinator. Areas of responsibility include supporting & implementing marketing and communications plans and programs that support effective messaging pertaining to Guelph CHC programs and Services as well supporting any work related to projects on Corporate Team workplan.

You are a current student or recent graduate in the area of Marketing, Communications or related field or have an equivalent combination of education and experience.  You have a high skill level and experience with MS Office suite products such as Word, Excel, PowerPoint and Outlook and experience with Adobe Creative Suite is an asset.  You are knowledgeable and have experience in marketing & communications, public relations and communications strategy development and proven experience in the area of social media: website, facebook, twitter etc. You also possess experience promoting and supporting special events.  You are able to multitask, work independently, set goals and produce results in a set timeline. You have excellent written communication and you understand and are committed to the Mission, Vision and Values of the Guelph CHC.

JOB TITLE:   COMMUNITY HEALTH PROGRAM ASSISTANT

EMPLOYMENT STATUS:   Contract  

HOURS/WEEK: 10 hours/week

As a member of the Community Health team, the Community Health Program Assistant will provide administrative support to Community Health Manager and Team including data entry, creating templates and documents, scheduling meetings/appointments , assisting with programs and services, and other related duties.

You have a minimum of 2 years of administration experience and have a high level of proficiency with Microsoft Office software; outlook, word, excel.  You have experience and understanding of community development principles and strong customer service skills especially as they related to our priority populations.  You are able to set goals and get results within a set timeline and are able to multi-task a variety of tasks while working independently.  You are an effective communicator, both written and verbal, have a high level of confidence and are a team player and are supportive of interdisciplinary practice.  You understand and are committed to the Mission, Vision and Values of the Guelph CHC.
