JOB TITLE:   Social Worker

EMPLOYMENT STATUS:   Contract (7 months)

HOURS/WEEK:   35 hours per week

As a member of the Community Health team, the Social Worker is responsible for providing counseling, family and group therapy within a holistic, health promotion framework. The Social Worker sees clients of the Guelph CHC, with both scheduled and urgent appointments, in office, and home visits in the community, during regular and extended hours. This role is responsible to; 1) maximize the functioning and independence of clients by providing psychosocial counselling services to individuals families and possibly groups, 2) ensure clients receive community support and advocacy, 3) facilitate referrals, 4) participate in case conferencing and provide consultation on psychosocial issues for Guelph CHC staff, 5) maintains client records, assessment notes and correspondence within the safeguard of confidentiality. 

You have a Masters of Social Work coupled with two to five years experience in a community based human service organization and have thorough knowledge of, and proficiency in, current assessment, psychosocial counseling techniques and group work skills, as well as, an understanding of community resources, preferably in Guelph.  You possess the ability to set goals and get results within a set timeline and multitask and work independently.  You have excellent written and oral communication skills and effective time management, decision making and organizational skills.  You are a good team member, are supportive of interdisciplinary practice, have a high level of confidentiality and are flexible towards change.  You understand and are committed to the Mission, Vision and Values of the Guelph CHC.

JOB TITLE:   Medical Secretary

EMPLOYMENT STATUS:  1 year contract

HOURS/WEEK:   35 hours per week

 The Medical Secretary is responsible for the coordination of new client intake and support for registration and data entry for group programming;  Completion of insurance and other claim forms (e.g. WSIB and ODSP) as well as letters to employers and other 3rd parties (e.g. lawyers);  Maintaining and updating specialist referral binders and electronic rolodex; Monitoring and troubleshooting process issues for electronic lab reports;  Ensuring new staff are setup for electronic lab process; Recalls for clients facing attrition;  Referrals including facilitation of external referrals, client notification and required document completion; Back up reception duties when needed.

You have a medical secretary diploma and a minimum of five years administrative medical experience in a primary health care setting supporting clinical providers (MDs, Nurse Practitioners, Physician Assistants).   You have excellent computer skills, including MS Word, Excel and Outlook with a minimum of 60-70wpm.  You have experience working effectively with culturally and economically diverse populations, and demonstrate excellent communication and interpersonal skills.  You are a self-starter with superior attention to detail and are able to work within set timelines, multi-task and work independently.   You have effective written/oral skills and decision making abilities. You understand and are committed to the Mission, Vision and Values of the Guelph CHC.

The Guelph Community Health Centre is looking to fill the following Full-Time position: Family Physician, MD (full-time)

The Guelph CHC provides innovative primary health services in an exciting and dynamically changing work environment.

Using an interdisciplinary approach, you will maximize your skills in delivery of the following:

  • Provide primary health care services, including comprehensive assessment and treatment, supportive client counseling, prenatal and neonatal care, and clinical consults with Nurse Practitioners.
  • Work within a determinants of health framework which emphasizes health promotion and education, disease prevention and recognizes the special risk factors (social, physical, psychological etc.) which may affect the health of clients.
  • Maintain professional development through continuing education, including Practice Based Learning opportunities.

You are able to work effectively, both independently and interdependently to provide comprehensive client-centred care. You possess strong organizational and analytical skills, and demonstrate a sound knowledge of the Ontario Health Care System and the Community Health Centre model of care.

You are a licensed Family Physician, as per the requirements of the Ontario College of Physicians and Surgeons, with CCFP certification.
